What are the risks of stem cell therapy?

Short answer

The most common risks are those of any injection — pain, swelling, and stiffness at the site for several days, with less common risks of infection, bleeding, and nerve or vessel injury. Beyond that, the honest answer is that long-term risk for orthopedic cell therapy is not fully characterised, because these uses are investigational. There is also a real financial risk: this is an expensive, generally uninsured treatment with no guarantee of benefit.

Procedure-related risks

Injection-site pain, swelling, and stiffness for a few days are common and generally settle. Infection, bleeding, and injury to nearby nerves or vessels are uncommon but serious, and are the reason sterile technique and appropriate imaging guidance matter.

Where a collection step is involved, that site carries its own risk of pain, bruising, and infection, and should be explained to you in advance.

The uncertainty that comes with investigational use

Because orthopedic cell therapy is not an FDA-approved indication, there is less systematic long-term safety data than there is for established treatments. Regulators have also taken action against clinics marketing unproven cell products, particularly for conditions well outside orthopedics.

Products vary widely between clinics in what they contain and how they are processed, which means safety experience with one protocol does not automatically transfer to another.

The risks that are not medical

Cost is a genuine risk factor here. These procedures are expensive, rarely covered by insurance, and carry no guarantee of benefit — so a poor response leaves you out of pocket with the original problem unresolved.

There is also the risk of delay: time spent on an investigational treatment is time not spent on an established one, which matters most where a condition is progressive or where surgery is genuinely indicated.
