How is BHRT different from synthetic hormone therapy?
Bioidentical hormones are structurally identical to the hormones your body already produces — same estradiol, same progesterone, same testosterone at the molecular level. Synthetic hormone therapies use molecules that are chemically related but structurally different, such as conjugated equine estrogens or medroxyprogesterone. The distinction matters because your body’s receptors were built to recognize its own hormones; a structurally different molecule can behave somewhat differently in the body, including how it’s metabolized and how it interacts with other tissues.

Why the molecular structure matters

It’s worth knowing that compounded bioidentical preparations — mixed by a compounding pharmacy to an individualized dose — are not FDA-approved as a category, even though the base hormones themselves are structurally identical to what your body makes. Separately, some body-identical hormone products manufactured at fixed doses are FDA-approved. Which route makes sense depends on your labs, your symptoms, and what your prescriber recommends.
